Activities/Services
The full-time social services director and full time activities director ensure a well rounded lifestyle for our residents, respectful of their ability to participate. We off a wide variety of activities seven days a week including bingo, social activities, special events, entertainment, arts and crafts, music therapy, and monthly resident council meetings. We have a shuttle van available for social outings, Dr. appointments and transportation to and from dialysis. The on site hair salon is convenient for residents to use.

Avalon Chandler Health Care Center proudly offers physical, occupational and speech therapy six days a week. We understand that each of our residents is an individual first. A person with different level of ability and different needs of rehabilitation.
Our geriatric rehabilitation program is designed by a dedicated team of health care professionals to motivate participants by building their strength and self-confidence of independence and ability to perform normal activities of day-to-day life.
Our goal is to return residents to their home, but we recognize that returning home may pose new challenges to both residents and their families. Our "Safe Transitions Program" provides patients with individualized plan of care to safely return to prior living arrangements for home care and additional services or equipment that is needed.
We care about the lives we change and are committed to the people who need us.
Building Description
This single level non-smoking center provides ample room for the many activities and functions planned for the residents including a private family room and serene, manicured courtyard with arbor and fountain for families to visit. Our building is licensed for 120 beds which include 12 private rooms which all have private showers in these r0oms. Our building has a designated 60 bed rehab unit and a 60 bed long term care unit.
All of our rooms are spacious and tastefully decorated with individual climate controls, individual color TV's in every room with cable, and telephones at bedside in our rehab unit.
